Maple Star Charity 5K - Registration Form

Instructions:  This is the official online registration page for the Maple Star Charity 5K Run/Walk Event.  Please provide the contact information below to complete an event registration for each participant or family (including the number of children and pets)...

Payment Information: You can pay for the Maple Star Charity 5K Event Registration in three ways:

Online via PayPal:

After you successfully submit the application below you will see a confirmation message appear and a PayPal "Donate" Button will be displayed that will allow you to process the $10.00 Registration Fee.  After processing your PayPal payment, an invoice will be sent to you via e-mail. If you have a PayPal account you can login and pay through your own account. If you don’t have a PayPal account you can pay as a GUEST using a Credit or Debit Card.

Check/Money Order

If you want to pay bu Check or Money Order, simply make you check payable to: Maple Star Colorado (please note “5K” on the memo line) and mail in your payment by October 31, 2011 to: Maple Star Colorado 2250 S. Oneida St., Ste. 100 Denver, CO 80224

Cash

Cash can be paid on the day of the race.
