Application Process
AVERAGE LENGTH OF TIME 3-6 MONTHS
INITIAL PACKET: Who is involved? What is required?
- Initial Application for Foster Home Licensing- this is a twenty four-page document to be completed by the two adults applying for foster home licensing.
- Friends and Family- 5-7 positive reference letters are required from people who have know you for a minimum of three years and no more than two letters can be from family members.
- Application for Treatment Parent Contract- this is an application with Maple Star Nevada.
- Individual Backgrounds-this is a form that each adult in the home will fill out about their family history and background.
- FBI- a national background check/fingerprinting is required on all individuals over the age of 18 living in the home to be licensed.
- Local Law Enforcement-local background checks are required on all individuals over the age of 18 living in the home.
- CPS- Child Protective Service (CANS) background checks are required on all individuals over the age of 18 living in the home.
- Department of Motor Vehicles- a copy of a valid drivers license or State of Nevada ID card is required for all individuals over 18 living in the home to be licensed for foster parenting.
- Social Security Administration- a copy of social security card is required for all individuals over the age of 18 living in the home.
- Marriage License/Divorce Decree's-If you are currently married a copy of your marriage license and a copy of any divorce decrees for previous marriages (if applicable) is required.
- Pets- if you have pet/pets copies of their current inoculations is required.
- Home Owners Insurance- homeowners or renters insurance is required on any home/apartment to be licensed for foster care.
- Car Insurance-Maple Star treatment parents are required to have a licensed and properly insured vehicle for transportation of children in care.
- American Red Cross-CPR/First Aid is required for licensure if you have a pool or spa on your property or live in a community with a pool or spa.
- Physician-If you have a pre-existing health condition you may be required to obtain permission from your physician before becoming a foster parent.
- Maple Star Home Study Process- approximately a 2-4 hour interview about you, and your partner/spouse/secondary caregiver, family background of all individuals in the home, and the ages/sex of children you are interested in working with.
- Maple Star Orientation- twenty hours of initial orientation and pre-service training is required for all adults in the home to be licensed for foster care.
- State of Nevada Division of Child and Family Services (DCFS) Home Licensing Inspection-After your application/ orientation/ home study process is complete with Maple Star your packet will be submitted to the DCFS office in your region (Reno, Elko, or Las Vegas) and a licensing worker will contact you to make an appointment for your home licensing inspection.
